Overview
An FS4 is a declaration submitted by each employee to their employer, detailing their personal tax status and specifying how tax should be withheld. It must be completed when starting a new job, or whenever there is a change in tax status (e.g. marriage or part‑time status). The employer uses information on the FS4 to apply the correct tax band (e.g. single, married, parent rates).
This article explains how to generate an FS4 from Talexio.
Permissions
You need to have the View/Manage All Employee Data and Generate Employee FSS Documents permissions to generate an FS4 from an employee's documents.
Generating an FS4
FS4s can be generated for employees from the Documents section of each employee's profile.

From the People section, search for the employee.

In their profile, click on the Documents section, click on Generate and click on FS4.
You will be given 3 options when generating the FS4:
- Download FS4 - With this toggle enabled, you will download a copy of the FS4.
- Save FS4 to Employee Documents - With this toggle enabled, you will save the FS4 to the employee's documents.
- Email FS4 to Employee - With this toggle enabled, a copy of the FS4 will be sent to the employee via email.

Click Generate and the file will be saved in the downloads folder of your device.
When the file is downloaded, it will not be complete. Section A - General Information and Section B - General Information will be completed based on the information in Talexio. The rest of the FS4 needs to be completed by the employee.
Uploading the Completed FS4
Once the FS4 has been completed by the employee, the document can be uploaded to the employee's profile following the steps outlined in the article here.
Implementing the FS4 is done during the completion of the employee's details and position.
