Overview
Question section is the core of your survey, providing the structure for gathering meaningful feedback from your team. While the surveys already come with a set of standard questions that cover essential topics, this section also allows you to add custom questions, ensuring the survey reflects your team’s unique needs and priorities.
Permissions
To create questions, will require the Manage Survey permission on global.
Themes
What is a Theme?
Themes is a useful feature allowing you to categorise the Survey Questions. Grouping Survey Questions into Themes ensures a logical flow for you, the respondent, and creates a clear framework for the survey, ultimately yielding better, more actionable data.
Creating a Theme
The process of creating a theme is shown below:
From the Surveys module, click on the Question Bank tab and click on Themes.
Click + Add Theme and provide the Theme Title and Description.
- Click Save & Add Another to save this Theme and create another one, or click Save & Close to save this Theme.
Now that Themes are created, you can proceed to create Survey Questions.
Survey Questions
What is a Question bank?
A question bank is a repository of questions which can be used in surveys, eliminating the need to create new questions from scratch each time, allowing you to pull questions from the bank to quickly build a survey. A list of standard questions is provided in the question bank. These questions aim to ensure consistency and cover essential topics, while still allowing clients to add custom questions to address their team’s unique needs.
Talexio questions may be hidden however these questions cannot be deleted.
Creating Custom Questions
Whilst the question bank is already populated with several standard questions, you can also create your own custom questions. This allows you to tailor the survey to your team’s specific needs and gather insights on the topics that matter most to your organisation.
To create a question:
- Go to the Question Bank
- Click on the "Add Question" button
- Fill in the question details
- Question: Add the question you would like participants to answer.
- Theme: Choose the category or topic this question relates to.
- Description: Provide a short explanation of the question purpose or additional context for respondents.

Response Type: Select the format for the response:
- Linear Scale: Choose the scale range (e.g., 5, 6, 7, or 10 points) and the label type from the dropdown. You may also enable reverse scoring if required.
- Text: Respondents can provide a free-text answer. Ideal for open ended or descriptive feedback.
- Multiple choice: Add multiple options for respondents, but only one answer may be selected.
- Checkboxes: Add multiple options where respondents can select more than one answer. Suitable when multiple responses may apply.
- Dropdown: Add several options that appear in a drop-down menu. Respondents can choose one answer.
- Click “Save” or “Save and Add Another” to add more questions.
Once a question has been saved, it will be added to the question bank.
Any question that has already been used in a survey cannot be deleted, and once a question has been saved, it cannot be edited.
